MaxsonMade Permanent Jewelry Waiver & Consent

Only one consent per person is needed unless you would like to change your response. If you receive additional permanent jewelry from MaxsonMade, you do not need to complete a new waiver. Consent will be linked to your customer account upon payment for your Permanent Jewelry.

Permanent Jewelry Disclosure & Care

Permanent jewelry involves welding a bracelet, anklet, necklace, or other chain directly onto the client without a traditional clasp for long-term wear. Permanent jewelry is durable but not indestructible. Chains may stretch, tarnish, discolor, snag, or break due to normal wear, lifestyle activities, skin chemistry, environmental exposure, accidental pulling, sports, work activities, chemicals, water exposure, or other external forces. Client acknowledges that chain style, fit, and sizing are selected and approved prior to welding and accepts full responsibility for the jewelry’s care, condition, maintenance, and wear after application. Due to the personalized nature of permanent jewelry services, refunds, returns, or exchanges are not offered once the service is completed.

Repair & Removal Policy

Each piece is fitted, welded, and inspected prior to completion. If the welded connection point fails within 48 hours under normal wear conditions, MaxsonMade may provide one complimentary reweld at its discretion. Repairs may require an in-person visit to MaxsonMade or an upcoming event location. If removal becomes necessary for medical procedures, swelling, injury, work requirements, or emergencies, jewelry should be cut at the welded connection point using appropriate cutting tools. Jewelry should be removed immediately if it becomes caught, causes swelling, circulation issues, injury, or medical concern. Emergency personnel may cut or remove the jewelry if medically necessary. MaxsonMade is not responsible for damage resulting from removal, snagging, stretching, accidental pulling, emergency cutting, tarnishing, discoloration, or breakage after the service is completed.

Jewelry Materials Disclosure

Jewelry materials may include sterling silver, fine silver, gold-filled, solid gold, stainless steel, enamel, or other metals. MaxsonMade is not responsible for allergic reactions, tarnishing, oxidation, discoloration, or skin sensitivities caused by individual skin chemistry, medications, lotions, perfumes, sweat, or environmental exposure. Clients are responsible for disclosing any known allergies, sensitivities, or medical concerns prior to service.

Assumption of Risk & Release of Liability

Permanent jewelry requires the use of specialized welding equipment while the jewelry is worn on the body. Possible risks include minor burns, heat discomfort, skin irritation, allergic reactions, pinching, discoloration, snagging, breakage, or other unforeseen complications. By proceeding with the service, you voluntarily assume all risks associated with permanent jewelry and release, waive, and hold harmless MaxsonMade, its owners, employees, contractors, affiliates, assistants, event hosts, and representatives from any claims, liabilities, damages, injuries, losses, costs, or expenses arising from or related to the service, including claims arising from ordinary negligence, except where prohibited by law.

Health & Safety Confirmation & Medical Treatment Authorization

By signing below, you confirm that you are at least 18 years old or have parental consent, do not have any known condition that would make this service unsafe, and understand that sparks, heat, and specialized welding equipment are used during the procedure. Clients who are pregnant, have pacemakers, implanted medical devices, seizure disorders, or heightened sensitivity to flashing lights or electrical equipment should notify MaxsonMade prior to service. In the event of an injury or medical emergency during the service, you authorize MaxsonMade to seek appropriate medical assistance if necessary. You understand that you are financially responsible for any related medical treatment or transportation costs incurred.

Minor Consent (Required for Clients Under 18)

Photography & Media Release & Email use

I grant permission to MaxsonMade to photograph and/or record video of the jewelry application process and finished jewelry for marketing, advertising, website, portfolio, and social media purposes. Personal identifying information will not intentionally be shared without additional consent.

Legal Agreement

This agreement shall be governed under the laws of the State of Ohio. If any portion is found unenforceable, the remaining provisions shall remain in effect. Electronic signatures shall be considered legally binding.
